This is generally caused by a mismatch between semi-parsed wikitext at the
formatHeadings() stage of a full parse, and the text encountered in
extractSections(). There may also be bugs relating to the difference between
the regex in extractSections() and the parsing behaviour of
doHeadings()/formatHeadings().
