Description

There's a warning without any header text when uploading some (possibly most)
files to various mediawiki projects.

A filename that needs to be adjusted will show a regular 'name changed' warning, but a
pre-adjusted title such as "Los_Angeles_Union_Station,_front_entrance.jpg" shows the
warning buttons with no listed warning, as described.
