Users should be allowed to subscribe to followups and comments to a revision
without making a comment on the revision.

And ofcourse be able to unwatch in some cases.

Perhaps code comments could be integrated further though so that the normal watchlist can be used, as well as WhatLinksHere etc.

LiquidTreads does go into the linking system, see for example:
http://www.mediawiki.org/wiki/Special:WhatLinksHere/Talk:License_integration_MediaWiki

On that though, comments should be editable to some degree and automatically close any unclosed tags (tidy? seperate bugs..)

Krinkle, if I do the watch/unwatch api module, would I be able to get you to do something JS-esk to do the watch/unwatch?

Though, adding extra watchers is easy, removing watchers whom have commented etc is less so..

Maybe having a tracking table for list of current users.. So commentors, tags changers etc can remove their "watching"

Sure, no problem. Ping me when I can start :)
