Once upon a time, there was
http://commons.wikimedia.org/wiki/File:Flag_of_ASEAN.svg
It was deleted, but since it's still linked to from various Wikipedia language versions, it still shows usage in the "Global file usage" section. So far so good.

The image was re-uploaded locally on en-wiki, to
http://en.wikipedia.org/wiki/File:Flag_of_ASEAN.svg
where the "Global file usage" now incorrectly shows usage from other language Wikipedias.
