Reporting against MediaWiki 1.17wmf1

Steps to reproduce:

  1. Navigate to the special page

http://test2.wikipedia.org/wiki/Special:SpecialPages

  1. Click on "Usability Initiative preference switch" link
  2. Click on "Click here to turn off the new features." link
  3. Click "Turn new feature off" button

Following error is displayed::

A database query syntax error has occurred. This may indicate a bug in the software. The last attempted database query was:
(SQL query hidden)
from within function "PrefSwitchSurvey::save". Database returned error "1146: Table 'test2wiki.prefswitch_survey' doesn't exist (10.0.6.44)".
