Iterating over category members does not produce a complete list. Some entries are duplicated and many others are simply missing.

This can be reproduced when listing members of category 'Kategoria:angielski (indeks)' on pl.wiktionary.

Fixed in r83814, needs the schema change in r83812 to be done on the cluster before it can be deployed to WMF wikis.
