Current thumbs fail to update on new file version. The most current thumb shows an older version.

Bug 28613 apparently reoccurs

Example: https://commons.wikimedia.org/wiki/File:Introduction_to_the_RepRap_Self_Replicating_3D_Printer.ogv You should not see the Google Earth screenshot - it is only in the oldest version. Both newer versions are blanked out. This behaviour did also occur before I had hidden the oldest file version's content.
