Have a look at this:

File:BBC.png doesn't exist in enWP. In Commons, File:BBC.png redirects to File:Blackberry_Circle_pentacle.png and if you go to http://commons.wikimedia.org/w/index.php?title=File:BBC.png you correctly get redirected and see an subtitle stating "(Redirected from File:BBC.png)".
However, in local projects, you don't get that subtitle, which causes confusion.

Isn't this Bug 14117, supposedly fixed in 2009?
Even if it isn't, whatever the fix, it needs a regression test.
Although the first link

doesn't show "(Redirected from File:BBC.png)" the image
http://upload.wikimedia.org/wikipedia/commons/b/ba/Blackberry_Circle_pentacle.png
is shown on its description. So, the redirect kind of works. On the other hand, redirected images show up on [[Special:WantedFiles]] (Bug 35350), and on the tracking categories (such as [[Category:Pages with broken file links]])
