Looks like these are not all the same error:

2_big.ogg causes ffmpeg2theora to fail in 2 pass mode, ogginfo also reports errors on this file so could be because of that. Filed a bug with ffmpeg2theora to track this issue at https://bugs.launchpad.net/ffmpeg2theora/+bug/978832

The other errors where caused by a configuration error, $wgMaxShellTime was limited to 50 seconds cpu time. This has a higher value now and the failed transcodes have been reset.
