I just put it on Labs today, so you can reproduce it by visiting http://pronunciationrecording.instance-proxy.wmflabs.org/wiki/Special:PronunciationRecording?debug=false and http://pronunciationrecording.instance-proxy.wmflabs.org/wiki/Special:PronunciationRecording?debug=true in a browser that supports the Web Audio API. At the moment, that probably means latest Chrome.
The toolbar is supposed to be unhidden if your browser supports the Web Audio API, but it only works in debug mode for some reason. In production mode it stays hidden.

I don't think the starting / at https://git.wikimedia.org/blob/mediawiki%2Fextensions%2FPronunciationRecording.git/6e3b5696bb79c19b3a5e3191fd7c155a3a6bf29f/PronunciationRecording.php#L33 should be there. However, that does not seem to fix it.
