I don't know where the problem lies here, but in any case, that's outside the scope of this request. Can you let me know what you want to be done from the Bugzilla side, ie. answer comment 2?

This is out of scope for Bugzilla, as there is nothing needed from anybody else to "activate" it. It is an on-wiki custom script on one wiki and its author or maintainers should be contacted if there are problems to port it. Asking on https://en.wikipedia.org/wiki/Wikipedia_talk:File_Upload_Wizard might provide some help.
