The HTML formatter for EntityIds should generate links pointing to the respective entity pages. Since there are multiple formatters for EntityIds, the HTML formatter should take one of those as a paramter for generating the surface text, and should take an EntityTitleLookup for determining the link target.

Ideally, it would also take a Linker object, but Linker has a largely static interface.
