Description

Broken inspector

Steps to reproduce:

1.Open the VisualEditor
2.Go to Insert menu and Settings menu

Observed Result:
<visualeditor-mwgalleryinspector-title> and <visualeditor-dialog-commands> is appearing in the Insert menu and Settings menu respectively.
Also selecting <visualeditor-mwgalleryinspector-title> opens up a broken gallery inspector.
