Image move on Commons messed up: No file description page and only file

See https://commons.wikimedia.org/wiki/File:Armii_Krajowej_5_9.JPG and https://commons.wikimedia.org/wiki/File:Kłodzko%2C_Armii_Krajowej_5_9.jpg . It seems that Marcus Cyron tried to move from first name to second name (there was rename request), but somehow it is broken, and this happened. (Creating bug per discussion on #wikimedia-commons.)

In the second file, there is no file description page and only file, but second seems to be original image, comparing file size. However second file's history does not exist, I don't know what happened exactly. It seems that Marcus uploaded lower size image after this happened.

This might welcome a comment by Marcus to clarify steps to reproduce the problem...

Steps really need clarification here; otherwise hard to investigate.

There are reports of very intermitent move failures for images, probably caused by race conditions. There are probably no reliable steps to reproduce, but something that happens at random on rare ocassion.
